Smallbiz uses a product called www.jotform.com
(have a look at their website !)

This allows you to:
- Easily design your own forms
- Copy off their extensive list of templates
- Copy one of ours !
(We will give you design requirements/steps for integration with Smallbiz)

We also offer a service to create the forms or integrate a form for Smallbiz use (P.O.A)

You do not need to pay for JotForm we provide the integration.

To design your own forms you must register for a Jotform account - a free account allows you to have 5 different forms, talk to us if you are designing more than 5 Forms.

Ways to send your form

The created link to the document can be used in a number of ways:

You can sms/email the link from Smallbiz to the client (either thru marketing or appointment reminders) as part of the message
Set up an auto select and send system so your client automatically receives the SMS before their appointment
You can have instore IPads or tablets that have shortcuts to the links on their desktops
You can have a QR link printed in the reception area, the client scans it with their phone, and they then have the form to fill out!
The documents are tailored to work on PC’s, MAC’s, IPads, Tablets, Mobiles (Android & Apple)

The integration

Smallbiz has integrated with/into Jotform, so that when the client “Submits” a form, it will become available in the Smallbiz client card:

It will automatically create a Smallbiz client card (for those that don’t have one)
Or update the card of those that have fields set up such as name, phone numbers, address, emails, marketing permissions will automatically update the Smallbiz card!
The actual form will be available in the documents tab of the Smallbiz client card. -You can also add photos in a similar way… eg click on the form link on your IPad or phone,type the clients name, click take photo and then submit … The photo turns up in your Smallbiz client card !!
